My Life My Health – Living a Healthy Life with Chronic Conditions
 
My Life, My Health is an evidence based Chronic Disease Self-Management Program. This workshop, written by Stanford University, is offered two-and-a-half hours, one day a week for six weeks in community settings such as senior centers, churches, libraries, etc. People with different chronic health problems attend together. Through mutual support and success, participants build confidence in their ability to manage their health and maintain active and fulfilling lives.

The Chronic Disease Self-Management Program meets weekly for 6 sessions to discuss:
*Managing symptoms
*Medication “how-to”
*Working with your healthcare team
*Setting weekly goals
*Effective problem-solving
*How to relax & handle difficult emotions
*Tips for eating well and increasing activity
*Techniques to help manage fatigue & pain
*Effective communication
*Getting a good night’s sleep

Registration is required for this program and no walk-ins will be taken. Anyone with a chronic condition or is a caregiver for someone with a chronic condition is welcome to attend. Attendance at all six workshops is highly recommended.
